Summary: The Team Nurse, RN is responsible for providing care in assessing patient problems and needs.He/she will implement plans of care, advise patients on health maintenance and prevention, maintainclinical documentation, mentor other staff, and/or provide case management per protocol under thedirection of the Provider.Areas of Responsibility:Patient Care & Education* Performs patient care activities that are assigned within the scope of competencies* Collaborates with the Care team and Providers to increase understanding, involvement and toenhance the PHC patient experience* Performs comprehensive triage and advice (phone and walk-in) for adult/geriatric, young adult,pediatric, infant and pregnant/nursing mothers under the direction of licensed clinical staff* Processes medication refills in accordance with protocol* Provides immediate response for urgent triage (includes phone) and codes within the facility* Performs nurse visits under the direction of Provider (s) (includes, but not limited to): wound care;B12; Coumadin; Testosterone injections; Insulin and glucometer teaching; Depo; BP checks; sutureremoval; medication reconciliation; and patient education for chronic disease per protocol* Assists with various procedures* Performs medication reconciliation and adjustments per protocol and with support of Provider* Informs and educates patients on the significance of abnormal lab results relative to chronic diseaseand/or Public Health tracked illnesses in accordance with PHC approved educational materials as perprotocol* Effectively utilizes PHC approved educational materials to provide patient education formedications, wellness, disease management, procedures and processes under the direction oflicensed clinical staffDocumentation* Ensures a clean, orderly and functional work environment* Assists team manager to facilitate policy implementation and adherence by staff* Effectively participates in problem identification and resolution* Ensures proper care in the use and maintenance of PHC equipment and supplies* Promotes continuous improvement of workplace safety and environmental practices* Performs miscellaneous job-related duties as assignedCase Management* Performs indirect case management (includes, but not limited to): patient recalls; missed breast carefollow up; and new patient intakes* Provides direct case management for uncontrolled chronic disease with Provider directionTraining* Assists and leads staff trainings* Provides guidance to MA's doing non-appointment work* Mentors and orients new staff, students and other team membersExperience and Skills:Qualifications: To perform this job successfully, an individual must be able to perform each essential dutysatisfactorily. The requirements listed below are representative of the knowledge, skill, and/or abilityrequired.Education/Experience:* Two year degree in Nursing required; Bachelors and/or Master's degree is preferred* Equivalent to an RN or higher degree from an accredited school of nursing* One to two years of increasingly responsible nursing experience including experience in communitynursing preferredDesired Skills:* An energetic and creative individual with high standards and an appropriate professional image* Proven ability to handle multiple projects and meet deadlines* Versatility, flexibility and willingness to work within an ambiguous, fast-paced environment* Demonstrated resourcefulness in providing solutions and setting priorities* Effective interpersonal skills to work effectively with all levels within the health center* Strong verbal and written communication skills with demonstrated ability to communicate clearlyand effectively at all levelsKnowledge of:* Principles, procedures, methods and techniques of nursing and health care* Modern practices and principles of method disease counseling* A variety of community resources* Basic management practices and principles* Principles and procedures of electronic medical record keeping* Universal precautions and infection control practices* Pertinent Federal, State and local laws, codes, and regulations regarding health careAbility to:* Provide administrative and nursing orientation to staff* Assess and evaluate patient responses to service provided, treatment, and medication* Demonstrated leadership skills and a desire to continually enhance nursing practice andperformance* Prepare clear and concise documentation and reports* Respond to requests and inquiries from the general public* Apply basic time management and patient care coordination skills to effectively provide nursingservices* Establish and maintain effective working relationships with those contacted in the course of work,including service representatives and the general public* Plan and organize multiple work tasks, problem-solve and analyze information toward sounddecisions* Identify problems/issues, develop and evaluate options in order to implement solutions* Appropriately manage highly sensitive and confidential patient informationLicenses and Certifications:* Possession of an appropriate, valid California Registered Nursing License* Possession of, or ability to obtain, an appropriate valid California driver's license* Possession of, or ability to obtain, an appropriate valid CPR certificate* Possession of, or ability to obtain, an appropriate valid BLS certificate* Candidate must be able to successfully meet PHC's credentialing and privileging requirementsLanguage Skills: Ability to effectively present information and respond to questions and request s frompatients, co-workers, and others as necessary. Bilingual in English and Spanish, both written and verbalpreferred.Mathematical Skills: Ability to add, subtract, multiply, divide in all units of measure, using whole numbers,common fractions and decimals.Reasoning Ability: Ability to recognize problems, collect data and establish factsComputer Skills: High level of computer literacy and proficient in MS Office (Word, Excel and Outlook),electronic medical records and web applications. Reasonable accommodations maybe made to enable individuals with disabilities to perform the essential functions.* Movement within health center complex environment repeatedly throughout the day* Ability to write by hand, use phone, use computer keyboard and mouse extensively to performgeneral office functions* Manual wrist and finger dexterity and wrist-finger speed sufficient to perform repetitive actions forextended periods of time* Ability to effectively communicate by hearing continuously and speech in a manner which can beunderstood by a diverse population* Ability to give and follow verbal and written instructions with attention to detail and accuracy* Must have visual acuity near and far (20 inches to 20 feet), depth perception, field of vision, abilityto focus on an object, ability to identify and distinguish colors* Ability to sit or stand for extended periods of time (minimum periods of one (1) hour at a time)* Ability to frequently walk, bend, squat, stoop, kneel, twist, crouch, and climb with the use of a stepstool* Ability to grasp, hold and pick up and reach with hands and arms* Ability to frequently lift and or move up to fifteen (15) pounds* Ability to occasionally lift and carry up to thirty-five (35) pounds (periodic lifting of supplies orequipment)* Ability to coordinate multiple tasks simultaneously* Ability to travel to and from other health center sites and community locationsWork Environment: The work environment characteristics described here are representative of those anemployee encounters while performing the essential functions of this job.While performing the duties of this job, the employee is regularly exposed to Health Center areas and generaloffice environment. The work environment is a clean, well-lit, well-ventilated, and temperature controlledenvironment. The noise level in the work environment is usually moderate.
