We partner with PACE (Program of All-Inclusive Care for the Elderly) to help these programs care for frail seniors. The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible.---

CareLinx by Sharecare helps PACE programs with healthcare professional staffing and in-home care services including transportation to medical appointments, recreational activities, and other senior care services.


Job Summary:

CareLinx by Sharecare is looking for California licensed Registered Nurses (Wound Care) to join our team.

As an RN you will be responsible for assessing and monitoring wounds. Cleaning and bandaging wounds. Working collaboratively with other members of the PACE care team to determine if additional treatments or changes in care are required. Educating patients and caretakers on wound care, infection and injury prevention, and pressure ulcer (bed sore) care.


Qualifications:
- BSN degree
- Current state licensure as a registered nurse (RN - CA)
- CPR and basic life support (BLS) certifications
- Prior experience in wound care and Certification (If available)
- Prior experience in inpatient and palliative care (preferred) Stepdown Unit/Progressive care unit
