Physical Therapist, PT to join our team in a skilled nursing
facility.RESPONSIBILITIES:Conduct comprehensive evaluations of
patients to identify physical, functional, and rehabilitation
needs.Develop and implement individualized treatment plans in
collaboration with the interdisciplinary team.Monitored and
documented patient progress and adjusted treatment plans as
necessary.Educate patients and caregivers on exercises and
techniques to promote recovery and prevent future injury.Provide
hands-on therapy, including exercises, manual therapy, and
modalities as appropriate.Ensure compliance with regulatory
standards and company policies in all aspects of care
delivery.Participate in clinical meetings and contribute to a
culture of continuous improvement.QUALIFICATIONS:New
passer/graduates are welcomeWith Skilled Nursing or Long-Term Care,
experience is a plusPrevious experience as a Physical Therapist
(PT) is a plusPhysical Therapy Degree from an accredited
programState licensure or eligibility for licensure as a physical
therapistIs in good standing with all regulatory agencies and
licensing boards is requiredInterest in working with the geriatric
populationStrong knowledge of anatomy, physiology, and medical
principles related to physical therapyExcellent communication and
interpersonal skillsAbility to work effectively both independently
and within a teamStrong problem-solving and critical-thinking
abilitiesProficiency with electronic medical record (EMR) systems
