Location: Petaluma , CA Schedule: Full-time, Part-time, or PRN

individualized treatment plans Provide skilled physical therapy

injury Work with patients who have medically complex needs in a
post-acute/SNF environment Collaborate closely with PTAs, OTs,
SLPs, nursing staff, and families to ensure comprehensive care
Document progress accurately and timely to support quality care and
compliance Qualifications Licensed OTA in the state of California
or ability to obtain license Experience in post-acute or SNF
setting preferred, but new grads are encouraged to apply Strong
communication, collaboration, and clinical skills Passion for
