Job Description Job Title: Care Coordinator Job
Responsibilities: The Care Coordinator is an integral part of the
treatment team at Muir Wood. The primary purpose of the Care
Coordinator is to provide supervision and direction to the boys at
Muir Wood, provide structure and promote accountability, and to
ensure their safety and wellness. This is typically achieved by
building a rapport with the boys, having an understanding and
compassion for their struggles, and setting limits and creating an
environment that promotes healing. Several shifts available
7am-3pm, 3pm-11pm, 11pm-7am Essential Duties: Liaison for residents
of Muir Wood and treatment team. Responsible for paperwork,
charting, shift notes and documentation. Building rapport with
residents and providing support and structure as needed. Dispensing
medication and accurate documentation, pill counts, and ensuring
that proper protocols are observed. Other Duties Include:
Responsible for being aware of where all assigned residents are and
what they are doing at all times. Awareness and ensuring that
residents participate in residential daily schedule including
participating in, and providing support to residents, groups,
activities, and events. Provide transportation to residents for
community outings and meetings in company vehicle in a safe and
legal manner. Practice compliance with all policies, record keeping
requirements, checklist assignments, assigned documentation, and
any other assigned duties. Remain current and up to date on all
trainings and certifications required to maintain employment.
Participation in monthly staff meetings Observe and monitor
residents--- behavior and intervenes based on schedule, individual
treatment plans and house needs Documents observations and
interventions in resident records Documentation in Muir Wood shift
log including check in and check out as well as a summary of the
events during the shift Ensures transportation safety by conducting
van inspections following use of Muir Wood van and following
documented safety rules Ensures that resident telephones are
available during designated hours Supervision of
self-administration of resident medications per physician orders
and maintain training in medication dispensing per State of
California Community Care Licensing Ensures physical plant safety
and security Orientation of all new residents to policies of Muir
Wood Conducts introduction/check-in to Muir Wood with new residents
including search of all items prior to admittance and lock up of
contraband and medications One-to-one supervision of residents at
risk for AMA, suicidal risk, eating disorders, etc., per directive
of the Clinical Director Follows all emergency procedures including
paging protocol, following directives given exactly, transportation
protocol, etc. Active participation in intervening with staff in
milieu to troubleshoot and improve proper use of behavioral
intervention techniques specified in the behavioral intervention
plans. Documentation of services as required by applicable law and
regulation, and other duties as assigned to facilitate program
success and the ability of residents to benefit from programming.
Other duties as assigned by the Clinical Director Qualifications
Required Education/Qualifications H.S. Degree or GED required;
bachelor---s level degree in psychology, counseling or sociology
preferred. Prior work experience in other behavioral health
treatment settings with adolescents a plus. Must have a valid
driver---s license and be eligible for insurance coverage for
driving the company---s vehicles.' Company Description Muir Wood
Adolescent & Family Services (Muir Wood), is a Joint Commission
accredited, residential treatment program for adolescents with
substance use disorder and co-occurring mental health issues. Muir
Wood has 8 gender-separate programs, located on a total of 26 acres
in Sonoma County, CA. Muir Wood---s treatment team includes
full-time, board-certified psychiatrists, medical and nursing
staff, psychologists, and six full-time Masters-level, licensed
therapists. Our individual and family therapists carry a maximum
caseload of six clients. In addition, the Muir Wood Learning

of Schools and Colleges (WASC) accredited supplemental academic
