Job Description Hiring: Care Manager – Sunnyvale, CA A
fast-growing, mission-driven home care agency in Sunnyvale is
seeking a Compassionate Care Manager to join its team. If you have
a caregiving background or openness to caregiving , that’s a huge
plus! Most importantly, we’re seeking someone who is presentable,
empathetic, and passionate about supporting both clients and
caregivers . Role Highlights: Conduct client assessments and
develop personalized care plans Provide orientation and training
for caregivers (in office & one-on-one) Support clients and
caregivers through regular check-ins Match clients with caregivers
to ensure the best fit Be on-call and step in to provide care when
needed Visit clients approximately once a month for updates,
support, or training Fill in on shifts occasionally (about once a
